Patient Agreement & Terms of Service
By seeking dental treatment at Mercado Dental Studio, you agree to the following terms and conditions. Please read carefully and contact our office with any questions.
Patient Responsibilities
Patients agree to:
- Provide complete and accurate health and insurance information
- Inform our team of any medication changes or new health conditions
- Follow treatment recommendations and pre and post-treatment instructions
- Maintain good oral hygiene between appointments
- Keep scheduled appointments or provide at least 48-hour notice for cancellations
- Pay for services according to agreed payment terms
- Treat our staff and office with respect and courtesy
Appointment Policy
Patients are expected to arrive on time for appointments. We reserve the right to reschedule patients who arrive more than 15 minutes late. Cancellations must be made at least 48 hours in advance. Appointments cancelled with less notice or missed without cancellation may result in a cancellation fee. Your initial appointment is reserved exclusively for you and typically lasts one to two hours.
Payment Terms & Financial Responsibility
Payment is due at the time of service unless other arrangements have been made in advance. We accept cash, checks, credit cards, and electronic transfers. You are responsible for any portion of fees not covered by insurance, including deductibles, co-pays, and non-covered procedures. We offer flexible payment plans to help make treatment affordable. Past due balances remaining unpaid for 90 days may be referred to a collection agency.
Insurance
We file insurance claims as a courtesy to our patients. However, you remain responsible for verifying your coverage and understanding your benefits. Insurance estimates provided before treatment are based on available information and are not guarantees of payment. Benefit verifications are estimates only. You are responsible for any difference between our estimate and actual insurance payment. Insurance claims disputes should be resolved between you and your insurance company.
Treatment Disclaimer & Consent
While we make every effort to provide high-quality dental care using current techniques, we cannot guarantee specific outcomes. All dental treatment carries inherent risks, benefits, and alternatives, which will be discussed with you before treatment begins. We will obtain your informed consent before proceeding. Complex cases may require referral to a dental specialist outside our practice.
Post-Treatment Care & Instructions
Patients are responsible for following all post-treatment care instructions to ensure optimal healing and results. Failure to follow instructions may compromise treatment outcomes and longevity. If you have questions about post-treatment care, contact our office immediately.
Dental Records & Radiographs
Dental records, radiographs, and treatment photos are the property of Mercado Dental Studio but are available to patients upon written request. Records may be released to other dentists or specialists with your written authorization. Requests for records should be made in writing.
Limitation of Liability
Mercado Dental Studio is not responsible for lost, stolen, or damaged personal items brought to our office. Patients are encouraged to leave valuables, jewelry, and important documents at home or in a secure location.
Modification of Terms
We reserve the right to modify these terms at any time. Changes will be effective immediately upon posting to our website. Your continued use of our services indicates acceptance of any modified terms.
